Testosterone Replacement Therapy: Myths, Realities, and What Every Couple Needs to Know

Dr. Diane: Hey everybody, welcome back to another episode of the Libido Lounge. I’m your host, Dr. Diane and I’m so excited to dispel some myths and get to some of the truths behind testosterone today. There are so many misunderstandings about this hormone and so many people are using this hormone in isolation without understanding the whole totality of what we need to think about with men’s health and libido. So I am just absolutely thrilled to introduce to you guys all to Dr. Burnett and have this amazing conversation today.

Dr. Diane: Welcome to the show, Bud.

Dr. Burnett: It’s so great to be with you today. Thank you so much for having me, Dr. Diane.

Dr. Diane: Let’s jump right in. In your experience working with thousands of patients, how does testosterone affect libido? What have you seen when people are low? Does it fix everything or is it just one piece of the puzzle?

Dr. Burnett: Testosterone is linked with libido—sexual interest, sexual drive, and sexual desire. On the male side, androgens like testosterone have impact across cognitive function, physical functioning, and sexual functioning. In the area of sexual function, libido is really where testosterone has the greatest impact.

However, it’s a much more complex story than many assume. There’s this idea that testosterone is the be-all and end-all factor for sexual functioning in all respects, but it primarily affects sexual libido and may modify some other aspects of the male sexual response cycle.

Low Testosterone Symptoms in Men and When to Take Action

Dr. Diane: What signs and symptoms should men be looking for? When should someone actually get their testosterone checked?

Dr. Burnett: If you’re not functioning at the level you used to—especially with consistent loss of sexual desire—that’s when it’s worth seeking professional help. Start with your primary care doctor, who may refer you to a urologist or sexual medicine specialist. For true sexual libido or desire issues, we often screen with testosterone levels.

Dr. Diane: What about erectile dysfunction? How much does testosterone replacement therapy actually help with that?

Dr. Burnett: This is a common point of confusion. Many men think, “I’m not getting erections, I just need testosterone.” But it’s not that simple. Erectile dysfunction may have a host of factors involved—not just interpersonal relationships, but also mental health like depression and anxiety, and physical health issues such as diabetes or cardiovascular disease. Testosterone is more directly linked to desire than to fixing erections on its own.

Does TRT Help Erectile Dysfunction? The Honest Truth

Dr. Diane: So does TRT help erectile dysfunction in many cases?

Dr. Burnett: It can support overall sexual function when low testosterone is a factor, but we have to address the full picture. Simply starting testosterone deficiency treatment without understanding other contributors often leads to disappointing results.

Testing and Treatment: Testosterone Replacement Therapy Options

Dr. Diane: From a self-advocacy standpoint, what lab tests should men ask for?

Dr. Burnett: Start simple with total and free testosterone. If those are abnormal, we can go deeper with sex hormone binding globulin, LH, FSH, DHEA, and others.

Dr. Diane: What about treatment? Do you recommend DHEA as a precursor or go straight to testosterone replacement therapy?

Dr. Burnett: I tend to go with actual testosterone replacement therapy. There are a variety of therapeutic delivery systems, and we have more confidence in safety and effectiveness with direct testosterone.

TRT Injections vs Gel: Which Works Better?

Dr. Diane: Many men wonder about delivery methods. Have you seen certain ones work better? What about TRT injections vs gel?

Dr. Burnett: Both have their place, and it’s often about patient preference, accessibility, and insurance coverage. Injections can create a surge followed by a drop, while topical gels and creams offer more even administration. However, with topicals we have to worry about transference to partners. Injections may raise hematocrit levels more, so we monitor that closely.

Some patients love the highs from injections for energy and libido, while others prefer the steadiness of gels. We tailor it based on individual needs and concerns.

Testosterone Replacement Therapy Side Effects and Lifestyle Factors

Dr. Diane: What testosterone replacement therapy side effects should people be aware of?

Dr. Burnett: With injections, we watch blood counts and hematocrit closely. Lifestyle factors matter too—addressing metabolic syndrome, diabetes, cardiovascular health, and overall wellness can dramatically improve results from testosterone deficiency treatment.
