Dr. Diane: Hi everybody, welcome back to another episode of the Libido Lounge. I’m your host, Dr. Diane, and I have a real treat for you guys today. There are so many taboos. There are so many stigmas when it comes to sexual health and erectile dysfunction in men. And there’s so many men that suffer in silence, that suffer quietly, and their relationships suffer, their partnerships suffer. And it doesn’t have to be this way. So I wanted to bring on Sam Itshaki, also known as Mr. Get Bigger Now, because he is an expert in all of these things. So welcome, Sam. It is absolutely a pleasure to be with you today. Thank you for coming on the lounge.

Sam Itshaki: Thank you for having me, Diane. It’s a pleasure to be here.

Dr. Diane: So, let’s talk about we’re going to get into all the nitty-gritty on erectile dysfunction, how people can fix it, dispel myths, all of that. But before we do that, tell us a little bit about your history, how you got inspired in this project, and why you’re known as Mr. Get Bigger Now.

Sam Itshaki: I did not imagine that this would be my life’s mission. I kind of fell into it. In my 20s I was bored one day and I’m someone who loves to try new things. I consider myself a biohacker. So if there’s anything that can enhance who I am physically, emotionally, mentally—if the risk is low enough, I will try it. One weekend I decided to walk into an adult store for the first time in my life. I didn’t really know what I was going to get. I figured I’ll just explore, find something I like, a toy, and have fun over the weekend. And so I walk in and I see this giant penis pump. It was like this big. And it didn’t seem normal or realistic. So I was very curious about it. I ended up getting it. I put this thing on and before I tell you what happens, let me just briefly explain how pumps work.

Sam Itshaki: Pumps create a gentle vacuum pressure around the penis and that draws blood flow to the penis. I put this thing on, start pumping, and my penis just grows in the tube. I give it another squeeze and pump a little more, and it keeps going and going. I just look down and I’m like, I can’t believe this thing actually works because I really didn’t expect much from it. That first pump broke after a few minutes of use. I was a little disappointed, but more than anything, I was super excited to get a better one and figure this out. From there I got a water pump and ended up injuring myself. Couldn’t have sex for a week. Because it is important to know how not to injure yourself.

Sam Itshaki: It’s important to get the right product that is safe. It’s important to read the instructions—and this is for all the men out there who don’t. Myself included by the way. I realized I have to take this seriously. I can’t just try it and see what happens because I only got one penis in this lifetime. So I did hours and hours of research online. There are online communities of men who talk about penis enlargement and enhancement in general—not just enlargement. It’s about getting better rigidity, things like that. And all those men share their stats with the rest of the men on the forum, anonymously. They basically have a diary of their progress and how much bigger they’re getting over time. So I figured if I follow what the professionals do, if I follow their routine, read their tips and tricks, I’ll be successful at it.

Dr. Diane: It’s really cool. And I also want to comment that I love that your nickname is Mr. Get Bigger Now and not just Mr. Get Bigger. There are some instant results literally. So we’re talking about right now, which I think is a really key thing to mention. Now, I’m very curious—it’s not just about the instant result, it’s about taking action. It’s very easy to learn something new and then not apply it in your life. And I want to not only educate people and tell them what works and what doesn’t but also motivate them to take action.

Sam Itshaki: Absolutely. Clarity of what works is the most important thing. But it’s about taking action.

Dr. Diane: So let’s break that down. There are manual pumps, electric pumps, water pumps. How many different types are there? And what do people need to know? What can go really wrong when trying these things?

Sam Itshaki: About two years ago, I ended up designing my own pump from the ground up after trying 39 pumps. I really wanted to know the pros and cons of each one. I realized no one has good instructions. The best instructions would say put it on and start pumping. That’s not even close to enough. There’s supposed to be a routine. How often do I do it? What pressure do I need to use? When I was just starting, I didn’t know. I realized the guidance out there is terrible. No company was able to help with pumping itself. As the CEO of the Dr. Joel Kaplan company, I make sure that everyone who answers the phone has been using a pump for at least a year. They’re male enhancement coaches. Dr. Diane: That’s really unique that you can get a hold of somebody and actually learn the routine. So what should someone look for when they buy a pump? Why are yours better?

Sam Itshaki: The main goal is safety, and second is efficacy. First, don’t hurt yourself. You need a pump with a gauge. That is the most important thing. And a pump that is a medical device—there are so many cheap toys out there. Gauges are number one. Being a medical device means high-quality manufacturing and risk assessments. For efficacy, cylinder sizing matters. A lot of pumps are one-size-fits-all with an oversized cylinder and a squishy gasket at the base. That gasket constricts blood flow—it’s supposed to increase blood flow but something is constricting it at the same time. It makes those pumps less effective. You’re causing strain on the skin with very little blood flow going in.

Dr. Diane: If it’s restricting blood flow, is there a safety issue long term where we’re causing even more erectile dysfunction?

Sam Itshaki: You can restrict blood flow up to 30 minutes generally. Constriction rings—also known as cock rings—come with pumps. For men with erectile dysfunction, they use the pump to gain an erection, then roll down the ring to keep blood in the penis and maintain the erection for sex.

Dr. Diane: Beautiful. So that’s how you use the pump to treat erectile dysfunction short term. But it’s very different when you use a pump for long-term benefits like an exercise.

Sam Itshaki: My recommendation is up to -10 inches of mercury. On this pump, it’s level 10. The pump prevents going beyond a dangerous level.

Dr. Diane: Before we talk about the benefits, let’s talk about erectile fitness in general. There are two terms that get thrown around: erectile dysfunction and male enhancement.

Sam Itshaki: Erectile dysfunction is not an on-off switch. It’s a spectrum. In our 20s we get great erections—rigid, spontaneous, little stimulation needed. As time goes by, we lose erection quality, rigidity, and size due to decline in testosterone and nitric oxide production. Both decline with age. We have less blood flow. Plus people get busy, have less sex. Less of this “exercise” of getting an erection. Nocturnal erections decrease too. It becomes a cycle: less sex, less erection, atrophy sets in faster. The solutions out there—no one really talks about good solutions. Usually doctors prescribe pills. Pills are a band-aid. They mask the root cause. Pumps address the root cause because the number one cause of mild erectile dysfunction is blood flow issues. Blood vessels get narrower. Pumps are proven to reverse mild erectile dysfunction. They create new blood vessels through neovascularization. Pumps literally reverse atrophy. It’s an amazing exercise that more people need to know about and use over the age of 35.

Dr. Diane: Is erectile dysfunction inevitable? Largely yes if we don’t intervene. Age-related changes happen. Do you recommend men once they hit mid-30s throw the penis pump in as part of their health regimen to prevent those changes?

Sam Itshaki: 100%. Prevention is key. Just like brushing your teeth to prevent cavities or going to the gym to prevent muscle loss. It’s so much easier to prevent health problems. If you don’t do anything, the penis shrivels with age just like the rest of the body. Start early so you don’t have a problem later. And if you already have a problem, it’s not too late. Mild conditions can definitely be reversed.

Dr. Diane: What about long-term damage or side effects?

Sam Itshaki: Read the instructions. Rule number one. Rule two: there shouldn’t be any pain or discomfort. If you feel pain, reduce pressure or stop. If you’re healthy—no diabetes, no blood disorders—you can definitely use a penis pump. They’ve been used for decades, before Viagra or Cialis even existed. It’s a super proven treatment for safety.

Dr. Diane: You mentioned men seeing the penis grow inside. Are there permanent changes in size? Helping get some size back if they’ve lost it?

Sam Itshaki: Absolutely. I created an app to track progress because official sites said pumps don’t work for size, but I knew from experience and community data they do. We have about 12,000 users, over 60,000 exercise records. Success rate for positive results is 92%. Average gains are ¾ inch in length and about 20% in girth over five to six months. Dr. Diane: You said something about sensitivity. More feeling of pleasure?
